איך להתמודד עם דמנציה ואלצהיימר בקרב ניצולי שואה
מה קורה כאשר זיכרונות העבר מתעוררים מחדש? כאשר חיים שלמים, בהם גם חוויות טובות שנבנו מעל גבי הטראומה, נמחקים מהזיכרון והאדם נותר אל מול האימה שכה היטיב להדחיק? על התמודדות עם דמנציה ואלצהיימר בקרב ניצולי שואה
השלב הראשון של הדמנציה מאופיין בפגיעה ובדעיכה של הזיכרון לטווח קצר. זהו השלב שבו הזיכרונות הקדומים, אשר חלקם היו שמורים היטב בזיכרון לטווח ארוך, צפים ועולים. החולה עלול לשכוח מי יצא את דלת ביתו לפני רגע, אולם לזכור לפרטי פרטים את שנות ילדותו.
כאשר אנו מדברים על זיכרונות קדומים בקרב ניצולי שואה, אנו מדברים על שנים של אימה וזוועה, של זיכרונות קשים וכואבים שקשה לחיות ולתפקד בצידם. אצל רבים, הודחקו השנים האלה ואוחסנו במקום עמוק, בטוח, לטובת תפקוד וחיים מלאים ככל האפשר.
אך כאשר הזיכרונות האלה שבים ומציפים, נדמה שהניצולים חווים אותם מחדש: הם שבים אל החיים בגטו, אל אובדן הוריהם, אל המחלות, הרעב, הפחד והמוות המקיפים מכל עבר. לעיתים, הם חוזרים לדבר בשפת אמם, מה שמוסיף על קשיי התקשורת וההבנה של מצבם.
בשלבים מתקדמים יותר של המחלה, נוטים חולי דמנציה בכלל למלא את החורים בזיכרון באמצעות דמיונם ולהמציא סיפורים שנדמים להם ממשיים ומוחשיים, הכוללים גם הזיות ופרנויה. בקרב ניצולי שואה, אפשר להיתקל בחולים המשוכנעים, למשל, שהנאצים חזרו לשלטון, או שהם נמצאים במחנה השמדה, עדים שוב ושוב לאימת תאי הגזים. חולים אלה מתמודדים עם חרדה עמוקה, לילות נטולי שינה וימים מסויטים.
עבור אלה, אפילו התנהלות יומיומית שגרתית, כמו בדיקה רפואית או כניסה למקלחת, עלולה להיות קשה וטראומטית. אצל אלה אפשר לאבחן התנהגויות אובססיביות כמו אגירת מזון בשל פחד מרעב, סירוב להתקלח, התקף חרדה אל מול צורך בביצוע הליך רפואי. לעתים תהיה התגובה כרוכה בצעקות, בפרצי בכי, השתוללות ואף אלימות.
מתוך הבנה, ולו חלקית, של המתחולל בנפשם, אפשר להתכונן לתגובות הקשות ולהקל מעט על התמודדות החולים עם היומיום: בעת קבלת טיפול רפואי, יש ליצור סביבה רגועה ונעימה ככל האפשר ולהשתמש בשירותי מתורגמן בעת הצורך.
כדי להגביר את תחושת השליטה אצל המטופל מומלץ להימנע מהאכלה כפויה, ולהעדיף מתן תרופות דרך הפה ולא בזריקה.
במקום להתנגד לאגירת המזון האובססיבית, מומלץ להחליף את המזון המתכלה במזון משומר.
למניעת החרדה הנפוצה מירידה במשקל, יש לשמור על דיאטה מאוזנת.
כאשר החולה מוצף בזיכרונות, אין להתווכח איתו אלא לתת תוקף לתחושותיו ולנסות להרגיעו “בתוך הסיפור” שלו. ההתנגדות לסיפור רק תחריף את החרדה.

דמנציה זה לא סתם בלבול רגעי
לכולנו, בוודאי בגיל הזיקנה, יש רגעים של שכחה ובלבול. לא כל רגע כזה מעיד בהכרח על תחילתו של תהליך דמנטי. מה בכל זאת צריך להדליק נורת אזהרה?
1. כשאובדן הזיכרון משפיע על התפקוד
נשים לב לסוג השכחה ולתדירותה. אין מדובר בשכחה, שמתרחשת מדי פעם, של שמות עמיתים לעבודה או של מטלה כלשהי שנזכרים בה באיחור. אנשים הלוקים בדמנציה ישכחו לא רק את המטלה אלא גם את מטרתה, מקרי השכחה יופיעו לעתים קרובות, ובדרך כלל, הם לא ייזכרו במה ששכחו.
2. כשהמטלות היומיומיות הופכות קשות לביצוע
הפעלת מכשירי חשמל, תשלום חשבונות, קניות בסופר – כל אלה יהפכו לפתע למשימה מורכבת עד בלתי אפשרית. ניתן לראות גם ירידה ביכולת להתארגן, להתלבש, לבחור בגדים מתאימים, פעמים רבות לבישה של מספר רב של שכבות ללא פשר.
3. כשקשה לתקשר
כולנו שוכחים לעיתים מילה זו או אחרת, אך מוצאים מילה נרדפת או דרך אחרת לסיים את המשפט באופן קוהרנטי. הלוקים בדמנציה ישכחו מילים פשוטות, יתקשו למצוא להן את התחליף המתאים ולבנות – או להבין – משפטים מורכבים. בשלב מתקדם יותר של המחלה יהפכו המשפטים שלהם לבלתי מובנים לחלוטין.
4. כשיש קושי בהתמצאות במרחב ובזמן
לצאת אל הרחוב ולא לזכור את הדרך הביתה, להימצא במקום ולא לזכור איך ומתי הגעת אליו – זהו חוסר ההתמצאות במרחב, המאפיין את תהליך הדמנציה.
5. כשיכולת השיפוט נעלמת
בניגוד לטעות שנעשית מתוך חוסר ריכוז רגעי, אצל החולה הדמנטי עלולה להופיע שכחה מוחלטת של משימה או אחריות שהוטלה עליו, כמו למשל, שמירה על ילד. ירידה ביכולת השיפוט תבוא לידי ביטוי גם בניהול הכספים, ובמקרים רבים יהפכו הסובלים מדמנציה מטרה קלה לנוכלים המנצלים את כושר השיפוט הלקוי שלהם.
6. כשמופיעות בעיות בחשיבה מופשטת
קושי בעבודה עם מספרים או ביצוע פעולה מורכבת, כגון נהיגה. קושי לעקוב אחר חשבון הבנק וביצוע תשלומים שונים, או קושי בתכנון וביצוע פעולות בעלות כמה שלבים, לדוגמה בישול לפי מתכון בספר.
7. כששוכחים חפצים במקום לא רגיל

לעיתים קרובות יופיעו בחולי דמנציה שינויים משמעותיים באישיות. עם התקדמות המחלה, הופכים החולים בדרך כלל לאפאתיים כלפי סביבתם. אולם עוד לפני כן יכול אדם שהיה שקט ונעים הליכות להפוך לרגזן וטרחן, ולהיפך – אדם שהיה דעתן ולוחמני ייעשה פתאום לכנוע. ההשערה הרווחת היא שמדובר לא רק בשינויים הנוירולוגיים, אלא גם בהתמודדות הנפשית יאתם.
10. כשמפסיקים ליזום ומוותרים
הדמנציה מביאה איתה תסכול מחוסר היכולת הגובר לבצע מטלות, להתמיד בתחביבים או לשמר על קשרים. בהמשך, את מקום התסכול תופס חוסר אכפתיות והחולים מתקשים להניע את עצמם וליזום פעילויות.
אין דרך להחזיר את מה שכבר נפגע, אבל ניתן בהחלט לטפל ולהאט את קצב התפתחות המחלה. אפשר – ורצוי – לקבל תמיכה וסיוע הן לחולה, הן לסובבים אותו.

המלצות לסביבה בטוחה עבור הקשישים

בחדרי המגורים:

• קבעו את הטלפון במיקום נגיש ונוח. רצוי להשתמש בטלפון אלחוטי.
• אם אתם משתמשים בשטיחים – הצמידו אותם לרצפה באמצעות חומר הדבקה, כדי להקטין את הסיכוי להיתקל בשולי השטיח וליפול.
בחדרי האמבטיה והשירותים:
• רחצה במקלחון בטוחה יותר מאשר באמבט.

• מומלץ להתרחץ בישיבה, השתמשו בכיסא פלסטיק יציב בתוך המקלחון או באמבט.
במטבח
• מקמו בהישג יד את החפצים הנמצאים בשימוש יומי.
• בקשו עזרה בפעולות המצריכות טיפוס – בשום מקרה אל תטפסו על דרגש או על כיסא בעצמכם.
• בדקו את תקינות הכיריים ואת יציבותם. כמו כו, מומלץ להתקין בבית מאתר דליפות גז על מנת למנוע דליפות גז.
מדרגות
• הקפידו על תיקון מדרגות שבורות, כך שיהיו תמיד יציבות ושלמות.
• השאירו את המדרגות פנויות מחפצים.
• התקינו מעקה יציב, רצוי משני הצדדים.
• הקפידו על מדרגות מוארות היטב.
• מומלץ לצבוע את קצה המדרגה בצבע בולט.
תאורה
• הקפידו שתהיה חזקה מספיק, אך לא מסנוורת.
